Family Home Health Services helps clients improve health, manage self-care

By Jonathan Marsh, Owner

Kim Lowry, Family Home Health Services

There may be times when you or a loved one needs medical help in their home, such as after a major surgery. A company like Family Home Health Services is able to offer this kind of care. Family Home Health Services has 13 locations within 31 counties along the East and West coasts of Florida, including locations in Bradenton, Sarasota, and the Tampa Bay area. Its corporate headquarters are in Lakewood Ranch.

The professionals at Family Home Health Services provide skilled nursing, physical therapy, occupational therapy, and other health care services to clients within their home, says Kim Lowry, clinical coordinator for Family Home Health Services. Family Home Health Services prides itself on helping clients achieve their highest potential in day-to-day self-care activities, according to the company website.

“We offer another set of eyes and ears,” Lowry says. For instance, a doctor may recommend home health care to assist with wound care, diabetes care, or post-rehabilitation care. A nurse or therapist working with Family Home Health Services will visit the home typically two to four times a week. As the client improves, the visits will become less frequent. Health situations commonly addressed by Family Home Health Services professionals include newly diagnosed diabetes, congestive heart failure, pneumonia, and falls prevention. Medicare typically covers the cost for its services.

CareConnect

Under the guidance of Lowry, who has a background both as a nurse and administrator, a program was created at retirement community Water’s Edge of Bradenton under the auspices of Family Home Health Services to improve care for its residents. The program, CareConnect, offers 15 mobile physicians who visit Water’s Edge once a week for patient appointments. This helps eliminate transportation issues or mobility challenges that accompany leaving the facility for doctors’ appointments. Specialists who come to Water’s Edge include family doctors, dermatologists, cardiologists, and others. For instance, residents who use the CareConnect program even can have a skin cancer removed without ever leaving Water’s Edge. There’s a great convenience factor from having mobile physicians who visit Water’s Edge regularly, Lowry says. “They get to enjoy their life, and they’re not sitting and waiting in doctors’ offices,” she says.

Lowry also facilitates communication among the doctors and professionals at Family Home Health Services to help improve patient’s health and care coordination.
