Category 1 Benefits

You will be eligible for the following benefits starting on the first day of employment and will retain these benefits as long as are not inactive with the company for 30 days or longer. Health Advocate (A CoAdvantage Sponsored Benefit)

Call (1-866) 695-8622 for unlimited, confidential access to your own Personal Health Advocate 24 hours a day, 7 days a week! This service is available to you, your spouse/domestic partner, dependents, parents and your parents-in-law! You can call for assistance with finding doctors, resolving benefit issues, scheduling appointments, helping with eldercare, working with health insurance companies, helping to make informed decisions (researching conditions and treatment option and facilitating second opinions), assisting in transfer of medical records, and helping answer questions about test results, treatments and medications.

Employee Assistance Program (A CoAdvantage Sponsored Benefit)

Call (1-866) 570-3478 anytime, 24/7 for FREE professional help or guidance when you need it to solve life issues, whether personal or work-related. This service is available to you, your spouse/domestic partner and your dependents! The program offers professional counseling and consultation for employees and their families. The program has over 45,000 professionals such as attorneys, specialists, counselors, therapists, and psychologists to help.

Category 2 Benefits

You will be eligible for the following benefits after a benefit waiting period of 60 days after being hired as long as you are working an average of 30 or more hours per week. These are employer-sponsored plans, meaning they are offered to employees at relatively low cost. Note: The Flexible Spending Accounts are of no cost to you.

  • You can enroll:
  • After hire, after the 30-day benefit waiting period
  • During the annual open enrollment period
  • When there is a qualified change of status such as marital status.

  • Short Term Disability Income Protection (A CoAdvantage Sponsored Benefit)

  • Short term disability insurance replaces a portion of the employee’s income during leave from work for a temporary disability, such as pregnancy, accidental injuries, and illnesses.

  • Hospital Indemnity Medical (A CoAdvantage Sponsored Benefit)

  • Hospital Indemnity Medical insurance pays you benefits when you or your dependents are confined to a hospital, whether for planned or unplanned reasons. The plan pays cash directly to employees even if they don’t incur any out-of-pocket expenses. The payments can be used for any purpose, including medical copays, deductibles, and regular expenses such as food, rent, and utilities.
