Click Learn More for more information about the WellSky Personal Care mobile app. The WellSky Personal Care mobile app is the caregiver’s primary tool for viewing schedules, viewing client care assessments/care plans, clocking in and out of shifts, and documenting tasks/activities completed while on a shift. In addition, you can also find information on the WellSky Personal Care Online Portal, giving you an alternative to viewing schedules and client information via a web browser, such as on a home computer.

Overtime Authorization

Employees who anticipate the need for overtime to complete work assignments must notify their supervisor in advance and obtain approval prior to working hours that extend beyond their normal work schedule.
Caregiving Staff: A member of the admin staff will inform you and assist when the completion of this form is necessary.

CAREACADEMY TRAINING REQUEST FORM

Access to CareAcademy to complete quarterly training requirements will be requested via our CareAcademy Training Request form. Once approved for access, you will be given a 2-week period to complete the training. The periods are approximately from the 1st through the 14th -AND the 15th through the 28th. If you request access during a period that is already in progress, access may be granted within 2 to 3 business days. Please be sure to check with the office if you do not receive a response
