Under the guidance of Lowry, who has a background both as a nurse and administrator, a program was created at retirement community Water’s Edge of Bradenton under the auspices of Family Home Health Services to improve care for its residents. The program, CareConnect, offers 15 mobile physicians who visit Water’s Edge once a week for patient appointments. This helps eliminate transportation issues or mobility challenges that accompany leaving the facility for doctors’ appointments. Specialists who come to Water’s Edge include family doctors, dermatologists, cardiologists, and others. For instance, residents who use the CareConnect program even can have a skin cancer removed without ever leaving Water’s Edge. There’s a great convenience factor from having mobile physicians who visit Water’s Edge regularly, Lowry says. “They get to enjoy their life, and they’re not sitting and waiting in doctors’ offices,” she says.

Lowry also facilitates communication among the doctors and professionals at Family Home Health Services to help improve patient’s health and care coordination.
