Free Discharge Consult

We offer new and existing clients free discharge planning assistance. We will come to the hospital, rehab center or nursing home, and work with social workers and discharge planners to make your goal of returning home safely a reality. When the conversation starts about being released from the hospital with your medical team, we can help you consider the options that will allow you to go home. Call Home Helpers and we will help you work through the confusing process. We will be there when you need us most.

"Go Home" Discharge Package

Our team can provide you a safe and reliable way to go home.

In an effort to ease the transition for the entire family, Home Helpers offers a comprehensive discharge program. For a flat rate of $99.00, we will pick you up from the hospital or rehab center, bring you home, and provide 4 hours of customized service, including a safety assessment of your home. We will drive you home, pick up new prescriptions at your pharmacy, clean out the refrigerator, shop for fresh food, change the bed, prepare a nutritional meal, do some light housekeeping, and put follow up doctor appointments on the calendar. Our caregiver will make sure you are comfortable and safe and make a courtesy care call to the family. You can also customize the 4 hours of service to fit your individual needs.

A trip to the hospital can be an intimidating event for patients and their families. As a caregiver, you are focused completely on your family memberʼs medical treatment, and so is the hospital staff. You might not be giving much thought to what happens when your relative leaves the hospital.

Yet, the way this transition is handled—whether the discharge is to home, a rehabilitation (“rehab”) facility, or a nursing home—is critical to the health and well-being of your loved one. Studies have found that improvements in hospital discharge planning can dramatically improve the outcome for patients as they move to the next level of care.
