Why Home Helpers Home Care of Greater Milwaukee

WHY HOME HELPERS of Greater Milwaukee FOR in-HOME CARE Services?

There are over 160 home care companies in our 4-county service area. As a business, we need to articulate what makes us different.

Nurse Forward - and - Memory At Home

Our Compelling Approach to Home Care

Nurse Forward is a concept we developed nearly a decade ago when we realized that when families need our help, they truly value working with a Registered Nurse – a trusted health care professional who can help connect the appropriate home care plan to the underlying medical challenges that the client may be facing. Our nurses are involved with every client - and are responsible for overseeing all aspects of client care and communication with the families.

Memory at Home is a program that focuses on our team of Certified Dementia Specialists who are experts in the care of clients with dementia or other cognitive challenges. We certify our own Certified Dementia Specialists with nearly 70 hours of course instruction, testing, and weekly meetings to discuss key learnings and difficult cases. Layered on top of this expertise we have a Certified Dementia Trainer, a Certified Dementia Consultant, and several Certified Dementia Coaches who support our Certified Dementia Specialists and who are available to help families with particularly unique challenges. Our curriculum for Memory at Home is based on a collaborative effort between Teepa Snow, the industry's leading expert on helping individuals and families with dementia, CareAcademy, and our nursing team.

Our fundamental approach to home care is that everyone we help has a medical challenge. It might be physical, cognitive, emotional, or behavioral. It can be all of these - and it can run the gamut from mild to severe.

Who better to consult with for an assessment than a registered nurse who specializes in geriatric care?!

Our Nurse Forward patient approach is a registered home care business model with the State of Wisconsin where Home Helpers utilizes registered nurses to lead all areas of the patient engagement, including:

  • Conduct all patient assessments
  • Create and manage patient-centered care plans, including periodic visits to reassess patient status and what modifications are recommended. These are discussed with the patient and their families
  • Coordinate pre-hospital conferences to prepare for discharge home (planning ahead)
  • Coordinate hospital or rehab discharge care conferences to prepare for discharge home (planning for the new normal)
  • Schedule, attend, and transport patients to follow-up medical appointments
  • On-call 24/7 for emergencies
  • Our registered nurses are also Certified Dementia Practitioners

Our Nurse Forward approach is then married with our Integrated Wellness Program focusing on the whole person, including:

  • Safety: All staff are equipped with Personal Protective Equipment (PPE) and they have received additional infection control and COVID-19 training
  • Nutrition & Hydration
  • Physical/Exercise
  • Emotional/Social Support
  • Mental Exercise/Stimulation
