Vitals monitoring can be a simple way to help reduce the frequency that you or a loved one are admitted to the hospital. By tracking blood pressure, pulse weight, and oxygen levels, we can detect changes before they become a problem.

When onsite, our Trained Caregivers can accurately and consistently measure important vitals. We also offer telehealth services as well as a technology suite that allows you to capture health data and transmit it directly to your doctor. They can then monitor the findings.

Direct Link® Monitoring Services

Home Helpers Home Care is proud to offer Direct Link® products to help monitor and assist in vital signs measurement and reporting. The Direct Link® catalog offers a wide variety of PERS options. Some options include:

  • MXD/MXDF Landline, which is the most basic unit that offers monitoring abilities along with a fall button
  • Mobile Care Units, which are GPS-enabled, water-resistant, and have a long, 36-hour battery life
  • Mobile Help Solo Kits, which are GPS-enabled, works outside of the home, and are weather-proof wrist units that work with mobile devices

These are just three of the many options we offer. No matter what your loved one’s needs are, we’ve got a monitoring solution that will make sure they are always taken care of. Our Caregivers are thoroughly experienced with each of the models, can help you choose the one that best fits your needs, and can explain in detail how to use them.

Steps to Care

  1. 1.
    Initial Call Once we’ve received your information, we will have a short 5-10 minute call to answer any pressing concerns and set up a time to meet in person.
  2. 2.
    In-home Care Assessment Here we will answer all your questions, and ask some of our own to craft a Care Plan customized to your unique needs.
  3. 3.
    Start of Care We will come with our Caregiver on the first day of care to go through the Care Plan together and help with introductions.
  4. 4.
    Client Visits Once we have begun care, we will pop in at times our Caregiver is there to see how things are going and determine if we need to adjust the Care Plan.
