Daily peace of mind

The need to connect with someone is what makes us human and brings joy to the day. To help provide companionship or needed reminders, we have developed the Home Helpers Wellness Calls. These calls can ensure the client is well and can happen as often as you or loved one would like.

  • Morning or Evening Check-in
  • Medication Reminders
  • Hydration Reminders
  • Meal Reminders
  • Exercise Reminders
  • Companionship - A simple Hello - How are you today?

CUSTOMIZED RESPONSE PLAN

Should our Wellness Call go unanswered, we will work with you or your loved one to develop a customized response plan. We will determine how many times per day we will try to call. If we don’t get through, you can opt for family members to be notified via email or a call, so they can reach out. This closed loop process helps ensure the best care. If the situation isn’t urgent, let us know, and we will simply make a note of the missed call.

Steps to Care

  1. 1.
    Initial Call Once we’ve received your information, we will have a short 5-10 minute call to answer any pressing concerns and set up a time to meet in person.
  2. 2.
    In-home Care Assessment Here we will answer all your questions, and ask some of our own to craft a Care Plan customized to your unique needs.
  3. 3.
    Start of Care We will come with our Caregiver on the first day of care to go through the Care Plan together and help with introductions.
  4. 4.
    Client Visits Once we have begun care, we will pop in at times our Caregiver is there to see how things are going and determine if we need to adjust the Care Plan.
